AND NOW,
Petitioner, __________________ files this Petition for Relocation, and in
support thereof,
avers the following:
1. Petitioner is __________________

2. Respondent is __________________
(Mother/Father), Plaintiff/ Defendant, in the above-
captioned custody
matter, residing at __________________.
3. Mother and Father are currently
separated/divorced.
4. Mother and Father are the parents
of ____________ (number) minor children,

(names), who reside primarily with Mother/Father at
__________________.
5. Mother/Father
has primary physical custody of the children and Mother/Father has partial
physical custody of
the children.
6. Mother/Father
believes, and therefore avers, that it is in the best interest of the children
and
herself/himself that
she/he and the children move to __________________, subject to an
appropriate partial
physical custody schedule for Mother/Father.
7. The advantages of the proposed move
to __________________ are numerous. The move
will benefit the
children and Mother/Father financially, socially and emotionally and will
substantially improve
the quality of life for Mother/Father and the children.
8. Mother/Father's motivation for
moving is solely to improve the quality of life for the children
and herself/himself.
Mother/Father is not motivated in any way by a desire to frustrate the
relationship between
Mother/Father and the children.

is close enough to Mother/Father's home so that a custody
arrangement can be
established that will allow Mother/Father frequent and substantial contact
with the children.
The children will have an opportunity to continue and maintain a meaningful
relationship with
Mother/Father, and Mother/Father will have the opportunity to share in the love
and rearing of the
children.
10. Mother/Father's proposed move is not the result of a momentary
whim; rather,
Mother/Father has
carefully considered the options that are available to her/him and the
children.
11. The __________________ area provides excellent opportunities to
Mother/Father to
advance his/her
career because: Mother/Father has been offered a better job; there are many
more job
opportunities; she/he will have the opportunity to further her/his education;
there are
appropriate schools
for the children.
12. These opportunities are not
available where Mother/Father and the children presently live.
13. Mother/Father has friends and/or
family in the _____________________________ area.
14. Mother/Father would like to
relocate with the children by __________________.
15. If relocation is granted,
Mother/Father is willing to expand the current custodial
arrangements and
shall pay the transportation costs for the children for their time with
Father/Mother.
16. The children's needs are
inextricably bound to the health and happiness of Mother/Father.
The children's
welfare cannot be severed from that of Mother/Father, with whom they reside
on a primary basis,
and upon whose well-being they primarily depend.
WHEREFORE,
Mother/Father respectfully requests that this Honorable Court enter an Order:
(1) awarding the
parties shared legal custody; (2) granting Mother/Father primary physical
custody and
authorizing Mother/Father to relocate with the children to the _______________
area; and (3) granting
Mother/Father an appropriate partial physical custody schedule.
